You can use the SeeCare site to address your needs/goals/conditions, coordinate your care or the care for people you support, find information about your needs and goals, and share information with others. Using SeeCare you can do the following:
In the following sections you can click on the referenced page to go to that SeeCare page in your account or you can press the "(?)" for further information about that page.
On the Profile tab make sure that your personal details are up to date. If relevant this should also include the services that you provide and your company details.
If you are using this site in a professional capacity then on the Company Information page you can add the details of your company. You can add the details of the services you and your company provide on the Services page.
Public information will be used by others to find you and ask you to join their network and provide them with care. Personal details may be used to provide you with relevant information. The public profile is included in the Public Profile page.
You can also call a friend from their Public Profile Page using Skype. Simply press the call button near the friend's name. The call button will only be enabled if you and your friend have added your Skype Names into your respective profiles and your friend has granted you permission to contact them over Skype. Such permission can be granted by your friend on their Permissions Page or when they view your Public Profile Page.
Before you start you need to identify the needs/goals/conditions and record them in the Needs/Goals/Conditions page which is accessible from the Profile tab.
If you can not find the need/goal/condition in the existing list you can add new need/goal/condition by entering them into the text box. You will need to press the "Add" button to add the selected need/goal/condition. You can also change the exact name of a need/goal/condition using the "edit" button.
The account holder and their Care Coordinators can update a user's needs/goals/conditions. With the appropriate permissions a plan manager can decide on the Supportive Care Plans that address any of the needs/goals/conditions.
Once you have added a need/goal/condition the system will identify information resources that are relevant to that need/goal/condition. If such information is available you can see this information by pressing the "Relevant Information" link.
You and your Care Coordinators can add further information relevant to this need/goal/condition from the Information tab.
To develop a Care Plan you should first investigate the care and action options available and the possible carers that could support the plan. At this point you should learn from the experience of others and consult the relevant experts. This process is likely to be iterative until a suitable plan is determined.
You can use the Information tab to find information relevant to your needs/goals/conditions. Subject to permissions, you can also view information uploaded by others in similar circumstances. The information provided is identified as relevant to particular needs/goals/conditions, particular care plans, or generic information. Such information is also identified as site wide information or information which is personal to you or the people you care for.
You can also find care providers that provide relevant services using the Search People page.

However SeeCare does not generate, endorse, or recommend any such resource, information, comment, or experience generated or shared by others. SeeCare does not confirm the qualifications, accuracy, or any other statement or claim made by users sharing their experiences. You accept the sole responsibility for using information obtained using SeeCare. SeeCare recommends that you obtain independent expert advice before making any care decisions.
You should consider the appropriate plan for the needs/goals/conditions and who should be involved in delivering the care. Deciding on the plan might require some expert consultation. Please seek appropriate advice from experts.
Alternatively, and with appropriate permissions, you or your carers can use SeeCare to create an appropriate care plan as per Step 5 below.

Repeat steps (3.1) to (3.2) until suitable care is determined and care plans identified.
Your network includes your family and friends, people that support and care for you, and people that you support and care for. If there are other friends and carers that are not yet part of your Friends and Carers Network on SeeCare you should invite them to join your network.
You can search for a friend using the Search People page available through the Friends and Carers tab. If found simply press the "Add" button against their name. This will send them an invitation. When you invite someone to join your network you can specify which of your groups they belong to. You can also add them to one of your groups later.
If the friend or carer you would like to add to your network has not yet joined SeeCare you can send them an invitation e-mail using the Invite a Friend page available through the Friends and Carers tab. Upon joining SeeCare using your invitation they will have the opportunity to invite you to become their friend on SeeCare. Alternatively, after they have joined SeeCare you can add them to your network as per step (4.1) above.
If the friend or carer you would like to add to your network has not yet joined SeeCare, you can send them an invitation e-mail using the Invite a Friend page available through the Friends and Carers tab. You can also connect your SeeCare account with your Facebook account and find and invite your Facebook friends using the Account Setup Wizard.
Upon joining SeeCare using your invitation they will have the opportunity to invite you to become their friend on SeeCare. Alternatively, after they have joined SeeCare you can add them to your network as per step (4.1) above.
You can view the friends and carers in the Friends and Carers tab. Care Coordinators are identified by the icon beside their name. Here you can group your friends and carers in one of the predefined groups or in groups that you create. To add a friend to a group press the "Change Group" button near the friend's name.
Subject to appropriate permission, you can view the friends and carers of one your friends or people you care for, including the way they groups their friends and carers. To do this simply change the "selected person" at the top of the Friends and Carers tab.
To allow someone else to view your friends and carers you or your Care Coordinator will need to provide them with the Circle of Support permission on the Permissions page, which is available from the Profile tab. Your Care Coordinator always has your Circle of Support permission.
You have the authority to manage and coordinate your own care. However, you can also assign the permission to coordinate your care to other friends or carers by appointing them a "Care Coordinator". Appointing a Care Coordinator means that this person will be authorized to view and manage all of your care, including: Needs, Care Plans, and Care Items; as well as Circle of Support and Update Permission for all of your plans.
Circle of Support and Individual Plan permissions cannot be modified once an overall Care Coordinator Permission has been granted. When a Care Coordinator appointment is removed all these permissions will still be in place however they can be individually modified.
Please note that the appointed Care Coordinator has the authority to modify your Circle of Support and your Care Plan permissions. They are not able to assign Care Coordinator permission to other friends and carers, only you can do that.
If you appoint a Care Coordinator you will be required to confirm the authority and responsibility that is assigned with this appointment before you can save these permissions. The appointed Care Coordinator will also be required to confirm the authority and responsibility assigned to them.
You can call a friend from their Public Profile Page using Skype. Simply press the call button near the friend's name. The call button will only be enabled if you and your friend have added your Skype Names into your respective profiles and your friend has granted you permission to contact them over Skype. Such permission can be granted by your friend on their Permissions page or when they view your Public Profile Page.
Once all the "pieces" are in place you can assemble your Supportive Care Plans. If you are someone's Care Coordinator you can also create their Supportive Care Plans.
Appointing a primary support and support team will send them an email and a SeeCare request to participate in this action. You can see these actions as “Pending” until the people requested respond.
You can also call the selected supporters using Skype by simply pressing the phone button near the supporter's name. The call button will only be enabled if you and your supporter have added your Skype Names into your respective profiles and your supporter has granted you permission to contact them over Skype. Such permission can be granted by your supporter on their Permissions page or when they view your Public Profile page.
Sending a primary notification and support notifications will send them an email and a SeeCare notification to confirm this saved Care Status/Progress Note. You can see these notifications as “Pending” until the people notified confirm the notification.

Repeat steps (5.1), (5.2), (5.3), (5.4), (5.5), and (5.6) above until the required supportive care plans have been created. You will be able to update the supportive care plans at a later stage.
You can view your care calendar, circle of support (i.e., your friends and carers), the people you care for, and your care information using the Supportive Care Plans tab. Subject to permissions you can also view the circles of support and care information of other people in your network.
The Supportive Care Plan tab include a list of Friends and Carers with their needs/goals/conditions and care plans. The list includes all your friends and carers (including people you care for), grouped according to your groupings, and the care plans for which you have at least a view permission.
You can record the completion of actions and track the compliance with the planned care and support activity. When an action is complete simply click on any of the scheduled actions on any of the SeeCare calendars or action lists from your computer, tablet, or smart phone.
You can unmark an action indicating it is not complete by simply clicking on the action again.
Care Coordinators are identified by the icon beside their name. Actions highlighted in dark green are actions for which you are a primary carer. Actions highlighted in light green are actions for which you are a co-carer.
Put a “tick” against the person, need/goal/condition, or specific care plan you would like to view and press “show selected” to show the actions on the calendar. You can select multiple people, needs/goals/conditions, and care plans to view partial, full, or combined care information.
The care information for the selected people will be displayed on the Care Calendar and on the Care Plan List.
On the list of Friends and carers you can click on the person, plan, or care item and view the details. You can click on an action in the calendar and view its details. You can click on a plan or a care item in the Plan List and view its details.
You can also view care information on the Home (Dashboard) tab. The calendar on the Dashboard shows the care activities for the week ahead. The plans are displayed below the care calendar.
Actions included in the supportive care plans can be of type "Medication". On the Medications tab you can find information about the medications taken (i.e., actions of type "Medication") by you or the people you care for. Use the Friends and Carers list to select which people and which supportive care plans should be considered. Press "Show selected" to confirm your selection. Access to care information is subject to the permissions afforded to you by other people. As with other care information you can change the perspective in which you are considering the information.
Circles of support and the care information for a user are subjective to that user. Subject to appropriate permissions you can place yourself in the shoes of one of your friends or someone you care for and view the circle of support and care information from their perspective (to the extent permitted to you).
The friends and carers and care information is thus presented from the perspective of a selected user. When you login the information is presented from your perspective (denoted by "Me!"). To change the perspective select that person from the drop-down list on top of the list of Friends and Carers.
To allow someone to view information from your perspective you need to provide them with "Circle of Support" permission from the Permissions page, which is available from the Profile tab.
To share information you can share your care plans with your friends and carers. Your Care Coordinator has the responsibility and authority to manage and coordinate all aspects of your care. As such they can view your circles of support and view and update all your care plans and uploaded information.
You can allow other users to view your Supportive Care Plans. You can do that by editing the Plan Permissions on each plan. Alternatively you can manage all plan permissions by going to the Profile tab and using the Permissions page to see and change the permissions for all the plans. You can decide who has access to which plans. Note that if a person is involved in a Care Plan as a carer they should be able to view that Care Plan.
A person that can view a supportive care plan will also be able to view the need/goal/condition that this care plan addresses. Note that a care plan can address multiple needs/goals/conditions and there could be multiple care plans for the same need/goal/condition.
Your Care Coordinators can always view (and update) all your supportive care plans.
You can allow other users to view your friends and carers and the way you have grouped them. You can do that by going to the Profile tab and using the Permissions page to see and change the Circle of Support permissions.
You, or your Care Coordinator, can upload your personal documents, test results, and information to your personal SeeCare record using the Information tab. When uploading the information you can determine which of your needs/goals/conditions and which of your supportive care plans is this information relevant for. The information can also be categorized as general to your care.
Your friends and carers that have permission to view one of your specific need/goal/condition or specific care plan will also be able to see the information relevant to that specific/need/condition or specific care plan. They will also be able to see the general care information.
Information relevant to a specific care plan includes the information directly relevant to that plan and all information relevant to the needs/goal/conditions that this specific plan addresses. The information relevant to a specific care plan can be accessed directly from that plan. You can access information relevant to your needs/goals/conditions directly from the Needs/Goals/Conditions page which is accessible from the Profile tab. All information can be accessed from the Information tab.
You can also use this utility to upload information about your care experience and share that with your friends and carers.
There are many more things you can do within SeeCare. Below are some of these things. To find out more please read the individual help files for each of the SeeCare web pages.
You can create care actions for yourself directly from the Supportive Care Calendar or from the Home (Dashboard). Simply select the time slot on the calendar for the action, right click to open the menu, and select "Add Event". A new action window will open allowing you to select the supportive care plan and fill in the action details.
Subject to permissions you can also create care actions directly from the Supportive Care Calendar for other people. To do this you will need tick the "View calendar by person in need" box and then select that person in the list of Friends and Carers and press "Show selected". Create a new care action directly from the selected person's calendar. When the new action window will open you can add the action to any of that person's plans for which you have update permission.
You can also create Care Notes and CSAS reports for yourself directly from the Care Plans tab. Select the care item you wish to add and press the "Add Item" button.
Calendar events appear on the calendar but are not linked to any Supportive Care Plan. They are part of a "General Plan" which addresses a "general need". The general plan is automatically created for you. You can see it on the list of Friends and Carers.
You can create calendar events for yourself directly from the Supportive Care Calendar or from the Home (Dashboard). Simply select the time slot on the calendar for the calendar event, right click to open the menu, and select "Add Event". A new action window will open. Selecting "None" as the care plan will add it as a Calendar Event. Otherwise select the supportive care plan you would like to add this new action to and fill in the action details.
Subject to access permission to the General Plan of other people you can also create calendar events directly from the Care Calendar for them. To do this you will need tick the "View calendar by person in need" box and then select that person in the list of Friends and Carers and press "Show selected". Create a new calendar event directly from the selected person's calendar. When the new event window will open you can select "None" as the care plan.
You can manage the access permissions to your General Plan, and to the General Plan of some one you are a Care Manger of, using the Permissions page.
You can export the care actions for the selected Supportive Care Plans from the list of Friends and Carers. On the list of Friends and Carers select the supportive care plans you wish to export and press "Show Selected". Now press the "Export Selected" button. The list of care actions from the selected care plans will be exported to an iCalendar file which you can save. This iCalendar file can now be loaded into your calendar of choice.
Supportive Care Plans can be classified as "Active" or "Archived". By default the list of Friends and Carers displays the "Active" plans. You can archive a supportive care plan by changing its status on the Supportive Care Plan page. To view the archived plans select "Show Archived Plans" list of Friends and Carers. To view the details of the archived plans you can select the archived plans to be viewed on the list and press "Show Selected".
Subject to permission you can view the archived plans of others. Like with any other plan these permissions can be set on the plan itself or on the Permissions page. Note that archiving a plan does not alter its access permissions.
On the Friends and Carers tab you can group your friends and carers in one of the predefined groups or in groups that you create. There are five predefined groups available to you: "All", "Family", "Friends", "Health Professionals", and "Unassigned". You can also create your own groups. You can add and delete groups using the Manage Groups page.
SeeCare Mobile is the SeeCare Web-App for your mobile device. With SeeCare Mobile you can manage care and support anywhere and anytime. You can access information wherever you are and orchestrate care and support on the move.
To access SeeCare Mobile simply visit the SeeCare website using the browser on your smart phone. For easy access you can save the SeeCare Mobile Web-app to your home screen as follows:
