Create and manage Supportive Care Plans

A Supportive Care Plan outlines the care and support provided to a person in need. It includes Care Items – actions, assessments, and progress notes/Care Journal – that address one or more needs/goals. A plan is implemented by a Circle of Support.

Create Care Actions (activity, care, medication)

A Care Action can be an activity, medication, or support action, which forms part of a Supportive Care Plan. It identifies when and where the action will take place and the people responsible for it. People responsible for an action must confirm the support request.

Create Circles of Support

Friends and Carers are part of our Circles of Support. Carers assist us in care actions and as coordinators of our care plans. Sometimes they coordinate all our care and sometimes they support us when we share our care information with them. We can also provide similar care and support to our friends and be their carers.

Tracking and Compliance

Care actions can be marked as complete and compliance with the planned care and support activity can be tracked. Simply click on the action on any of SeeCare's calendars or action lists from your computer, tablet, or smart phone, and mark the action as complete.

Appoint Care Coordinators

A Care Coordinator has the responsibility and authority to manage and coordinate all aspects of the care and support of someone in need. You can appoint care coordinators and you can be a care coordinator for someone else in need. All relevant access permissions are immediately granted on appointment and can be revoked if required. Care coordinators can also identify and record the needs and goals and create care plans for someone they care for.

Manage Permissions

Access to care information is controlled under strict permissions. Friends and carers can be granted permission to access and manage your care information. You can decide who can update and who can view your supportive care plans. You can also decide which of your friends can see who are all your other friends and carers.

View the Care Calendar

The Care Calendar provides a high level view of the care activities. Care and support actions can be directly recorded on the Care Calendar. A care action is tagged as "[Pending]" if the carers providing the support have not yet accepted their care responsibility for this action. Care Coordinators can view the care calendar of someone they manage.

View Medications

Providing or taking medications is a special type of care activity. The list of medications currently taken is provided with details on frequency and the person that has added or updated the medication activity.

Share a Care Journal with the relevant Circle of Support

Care Journal records the journey of implementing a specific Supportive Care Plan. Care Journal is specific and is shared only with the relevant Circle of Support - those people involved in a particular care plan or activity. You can decide who should be informed of each Care Journal. Using the Care Journal you can keep track of which of the carers has acknowledged the journal update.

Keep all friends and carers up to date with your General Status

The General Status is shared with all your friends and carers. You can update all the friends and carers with general status updates and significant changes to circumstances - when you go away, if you have to go to hospital, or when you are feeling better. You can also share your SeeCare status with your Facebook friends using Facebook from SeeCare.

Find, upload, and share Relevant Information

To assist you in making decisions you can find information, uploaded documents, and links to external websites. This information is relevant to your needs or the needs of someone you care for. Personal documents, such as test results, can be uploaded and shared within relevant Circles of Support. You can access relevant information exactly where you need it. When considering needs/goals or reviewing care plans, relevant information is made available to you at a click of a button.

Telecare with SeeCare

SeeCare provides Telecare capability. Using Telecare with SeeCare you can call a friend, a carer, or someone you support and care for. Subject to their permission, you can simply press the Skype Call Button from your SeeCare account and start a voice or even video call. You can also use Telecare with SeeCare to contact people engaged in a particular care and support activity and schedule a support call. With a press of a button you can permit or prevent others from contacting you on Telecare with SeeCare.

Facebook Connectivity

Using Facebook from SeeCare you can access your Facebook account. You can use your existing Facebook friendship network to expand and maintain your SeeCare support network. You can find and invite your Facebook friends to join you on SeeCare so that they can support you and you can support them. You can also share your General SeeCare Status on your Facebook wall.
